Tretinoin cream is a vitamin A derivative used for acne amongst other skin conditions. It requires the proper application and care to maximise its benefits and minimise its side effects.

What is tretinoin cream?

Tretinoin belongs to a class of medications called topical retinoids. It works by accelerating skin cell turnover which promotes the shedding of old skin cells to surface the fresh skin underneath.

How to use tretinoin cream?

It is best to avoid harsh exfoliating scrubs when using tretinoin. The face needs to be completely dry before applying tretinoin as applying it on damp skin can increase irritation. 

Step 1 – A pea-sized amount of tretinoin is enough to cover the entire face. Using more will not increase efficacy and lead to irritation. 

Step 2 – Tretinoin should be applied thinly on the skin at night before going to bed. Small amounts should be spread evenly on the forehead, cheeks, chin, and nose. Sensitive areas like the mouth, eyes, and nostrils should be avoided.

Step 3 – A hydrating moisturiser should be used to minimise dryness and skin irritation. 

The application of tretinoin should start slow as the skin adjusts to the acne treatment. Using it every 2-3 nights initially will minimise the side effects of tretinoin. Once the skin tolerates it, increase it to nightly use to gain the optimum benefits of tretinoin. 

Retinoids make the skin more sensitive to the sun so using a broad-spectrum SPF should be used daily even in winter. Individuals actively using topical retinoids or oral should wear protective clothing and seek shade when outdoors. 

Individuals using tretinoin should refrain from using hard exfoliating scrubs, alcohol-based toners, or anything with strong active ingredients. 

How long does tretinoin take to work?

Noticeable results appear after 6 – 12 weeks of treatment and fine lines and dark spots may take longer to improve. It is important to use tretinoin consistently. 

Side effects of tretinoin

In the initial few weeks, it is common to experience some dryness, peeling, redness, and irritation. This is often called the ‘purge period’ where the existing acne may worsen as the pores are cleared. The skin can have slight stinging and darkening or lightening of the treated areas. 

Serious side effects include excessive irritation, severe burning, or if skin becomes very sensitive to light. In this scenario, it may be best to seek medical advice on treatment continuation.

When to seek help

Tretinoin is a beneficial cream but it may not be for everyone. It is important to note some peeling, discomfort, and redness are expected but if you experience persistent peeling or dryness that becomes painful, it is best to seek medical advice. 

Though tretinoin takes time to work, if there is no noticeable improvement after several months, it may be time to seek alternatives. If the skin becomes worse or any underlying skin conditions worsen, it may be beneficial to tailor the treatment.
